Scaffold tag at the Port of Oakland was a death trap waiting to happen

Was doing a walkthrough at the Port of Oakland yesterday and spotted a scaffold with a tag dated 3 weeks past due and one missing tie-rod, has anyone else caught sketchy gear like that on their own sites or do we just keep flagging it and hope someone listens?
