Vent: Almost lost a finger to a coolant hose last Tuesday

I was running a Haas VF-2 last week at my shop in Cleveland. Everything was normal, running a batch of aluminum parts. Then I noticed a coolant hose laying weird on the floor, went to kick it out of the way, and it was under the chip conveyor belt. The belt grabbed my boot lace, yanked my foot into the gap. If I hadn't been wearing steel toes with quick-release laces, I'd have been stuck. Has anyone else had close calls with stuff like hoses or chips on the floor near moving parts?

Did you report that to maintenance or your supervisor after it happened? I'm curious because a lot of shops I've worked at would just brush it off as 'operator error' and not fix the real problem with the hose routing. Seems like that chip conveyor belt is a straight up hazard if a loose hose can pull you in like that. What did they actually do to secure the hoses after you got out?

@maxl93 but what if the guy just didn't secure his own hose right?

Read somewhere that maintenance logs showed that hose was fine the week before this happened, @maxl93.
