Got to ask, which rule was the hardest to actually fix once the auditors pointed it out? Was it just the logging, or something bigger they made you change?

Missing safety drill logs. The audit showed we didn't write down our monthly fire drills. Without those notes, we had no proof they happened. We made a rule to log them right after each drill. It feels like extra work, but it keeps everyone accountable. You're right, rules fade away when no one checks.
